RED LOTUS TANTRA INSTITUTE  (RLTI) PARTICIPANT MEMBER ACTIVITY CONSENT, DISCLOSURE AND DISCLAIMER FORM

I request that a practitioner of Red Lotus Tantra Institute perform (Native American/First Nations and or indigenous medicine and or healing, Thai Yoga/ Thai Massage/ Nuad Boran, Jap Sen Nuat, Tapping, Ayurveda, Yoga, Laying on of Hands/ Chirothesia, Tantra and or natural healing session, evaluation, therapy, consultation, etc.) and set up a program (or have sessions, etc.) for the purpose of (educating, sharing and practicing Yoga, Yoga therapy, Thai Yoga, Thai Massage, sharing love, compassion, joy and equanimity, as well as conversations regarding our mutual physical, mental, emotional or spiritual health and wellness including food, diet and any appropriate exchanges mutually agreed upon, using established procedures and methods approved by the Red Lotus Tantra Institute (also known as RLTI). I understand that sessions may or may not involve touch, physical pressure and facilitated movement, laying on of hands, emotional, mental, psychological and spiritual counseling, exercises and or the use of natural plants, herbs, oils, substances as well as the ritual, ceremony and or sacraments of the RLTI.

I understand that Thai Yoga or Thai Massage, Ayurveda or any of the healing practices we share, is not intended as diagnosis, prescription, or treatment for any disease, physical or mental. It is also not intended as a substitute for regular medical care.

I understand that all procedures, Thai Yoga, Thai Massage, Pancha Karma, Ayurvedic meditations, affirmations, energy exchanges, prayers, Yoga practices, therapies, sessions, protocols, Bio-Tapp/ EFT, Laying on of Hands, Tantra Yoga and or other natural, non-invasive, holistic practices, ceremonies and healing conversations are private practices conducted by authorized members of Red Lotus Tantra Institute which I am a member, and that I am not receiving these above referenced services as a member of the public.

I understand that RLTI is a Private Membership Domain, for the protection, establishment and practice of Native American/First Nations, Indigenous, natural and earth based ceremony, sacrament, ritual, healing practices, Thai Yoga, Thai Massage, Nuad Boran, Ayurveda, TCM, Homeopathy, Traditional Naturopathy, Nature Cure, Reiki and/ or any related traditional health, wellness and healing arts and sciences authorized under, practitioner, lay minister, Ordained Ministerial/ Pastoral Doctoral scope of practice..

I am an adult, aged 21 or older, legally and mentally competent to make informed decisions regarding my health, wellness and recreation. I swear that all information provided by me in support of receiving services from the above referenced practitioner are correct and true as a Pre-condition towards participating in any private membership activities.
